For audio direct from the DS4 I’d say grab some Philips Fidelio X2HR with a V-MODA BoomPro Mic, you’ll not need an Amp/DAC either (having recently tested these I can confirm the DS4 can drive them with no issues).

This splitter at the end, breaks this up into your mic in and your audio out, and you just simply connect it to the back side of this and then into the back of your computer or again audio interface, and then you've turned this beautiful pair of headphones Into a headset, so that's pretty much what you get with all of this. There is a bit of paperwork in the the boom pro box as well, but well there we are that's pretty much all of it.
